Case description

In a 15-year-old boy, transthoracic echocardiography apical view shows the hypertrophic cardiomyopathy and a fragment of the perforated anterior mitral leaflet, moving into the left atrium during systole with aberrant apical papillary muscle. Color Doppler recording shows two separated regurgitant systolic flows toward the left atrium. There are left atrial dilatation, small effusion behind right atrium and right atrial membrane swinging along the interatrial septum (chiari network?).
