Ureteric injury following a failed stent insertion. There is marked curving of the lumen at the site of resent injury. The initial retrograde approach fails to gain access beyond the stricture. On the contrary it causes more injury to the mucosa and there is a sub-mucosal leak. The antegrade approach through a middle calyx puncture initially fails also. The flexible ureteroscope is employed to actively guide the hydrophilic wire through the torturous stricture. The distal end of the guidewire is grasped and pulled out of the urethra. Then a 8fr JJ stent is placed/
