Case description

As a result of the incident on 6/27/03 John Smith suffers from permanent paralysis from the neck down and suffers from the following permanent impairments.
1.Lack of sensation and Motor Movement (C6 incomplete quad)
2.Psychosocial Needs hindered due to complete independence on others
3.Anxiety and Depression
4.Compromise in concentration and visual-spatial ability
5.Impotence
6.Chronic Insomnia
7.Impaired Mobility affecting all of his ADL's
8.Musculoskeletal compromise -Spasticity of extremities - Pain- Ineffective physical function
9.Compromised Safety due to limited physical function
10.Contractures requiring splinting and othotics necessary for daily tasks
11.Loss of Bladder control
12.Loss of Bowel control
13.Incontinence and Constipation
14.Autonomic Dysreflexia
15.Hypertension
16.Frequent Urinary Tract Infections requiring hospitalizations
17.Urethral Erosion secondary to long term indwelling foley catheter
18.Pain
19.Loss of independence
20.Skin breakdown and Pressure Ulcers
