These are posterior circulation aneurysms, making 20% of intra cranial aneurysms, highly challenging surgically with high mortality and morbidity. This lady 62 yrs presented with sudden sever headache (sub arachnoid haemorrhage) and lt 3rd nerve palsy. Cerebral angiogram showed two aneurysms - large basilary artery bifurcation+posterior cerebral P1 aneurysm. She underwent lt sub temporal approach via basal temporal craniotomy and microsurgical clipping /obliteration of both aneurysms simultaneously with excellent post op recovery with no new deficits.

Procedure performed by dr Suresh Dugani/HUBLI/INDIA.
